New Patients

How to register

To register at Leeswood Surgery you will need to call at reception and collect a registration form and the associated documents required for joining the practice.  Once complete, these forms will need to be returned to the surgery in person.  Potential patients over the age of 16years will need to bring one form of  photographic ID such as passport or driving license, along with a utility bill or bank statement or council tax bill.   If you are prescribed repeat medication by your current Doctor, we will also require a copy of your repeat prescription slip for your records.  Please ensure you have enough medication from your current GP when registering with us, to cover the transition period of 2 - 3 weeks.

We are a dispensing practice, so all your medication will be dispensed from Leeswood Surgery and you will need to collect your medication during the dispensary opening times.
