How to make an appointment                

 

To Register at Leeswood surgery you will need to complete the necessary registration forms and surgery documents and ensure they are signed by the patient/guardian/carer, you will need to provide 2 forms of ID; one photographic and a utility bill or bank statement to confirm your current address.  You are also required to attend for a New Patient Medical appointment with the Nurse or HCA.  The Registration is usually complete within 2 weeks, so please ensure that you have enough medication from your previous GP to last before registering.

We have a zero tolerance of non attendance for appointments that have been booked and not cancelled.  If you fail to attend 3 booked appointments without cancelling or notification, your registration at Leeswood surgery will be at risk.  You may be removed from our patient list and be asked to register at another practice.  We will write to you to advise you of this, there may be a very good reason why you failed to attend or cancel and we will consider your reasons before making a decision to remove you.

Appointments can be made by phone on 01352 770212 or by calling in at reception. 

You can order medication by downloading the new NHS App and registering an account.  Children under the age of 16 cannot have an online account until they are able to manage their own medication.

We offer text reminders for booked appointments if you have registered a mobile number.  You can also CANCEL appointments upon receipt of your text reminder by simply replying CANCEL in the text field.

You can book appointments on the day or up to 4 weeks in advance for the Doctor or Nurse.

When booking appointments, you will be asked by the Receptionist for the reason for your call so that we can prioritize your need.  Please do not be offended by this, all our staff are trained and will treat any information provided in a confidential manner.

Please let us know as soon as possible if you no longer require an appointment that you have booked, so that we can offer it to another patient.  Failure to cancel will result in increased waiting times for appointments at the Surgery and a waste in NHS resources. Thank you.

Our Staff are very important to us, we do not tolerate any verbal or physical abuse or aggressive behaviour, they are only doing their job.  If you are abusive in any way towards any member of staff you WILL be removed

Face to face appointments are available with Dr Kumar, Dr Roopa, Dr Lodhi and the Advanced Nurse Practitioner and Practice Nurse.  We also have telephone triage appointments available with the Doctors.  Please ensure your telephone numbers are u to date.
